To be valid under 17 U.S.C. § 512(c)(3), your DMCA takedown notice must include:
- Your full legal name, mailing address, phone number, and email address.
- A description of the copyrighted work(s) you claim has been infringed.
- The exact URL(s) or location(s) of the infringing material on toolkot.com.
- A statement that you have a good faith belief the use is not authorized by the copyright owner, its agent, or the law.
- A statement that the information in your notice is accurate and, under penalty of perjury, that you are the copyright owner or authorized to act on their behalf.
- Your physical or electronic signature (typing your full name constitutes an electronic signature).

04 — Counter Notice
Filing a Counter NoticeIf you believe content was removed in error
If you believe your content was removed due to a mistake or misidentification, you may submit a counter notification under 17 U.S.C. § 512(g). Your counter notice must include:
- Your full name, address, phone number, and email address.
- Identification of the removed material and its location before removal.
- A statement under penalty of perjury that you have a good faith belief the content was removed due to mistake or misidentification.
- Consent to jurisdiction of the Federal District Court for your area, or if outside the U.S., any judicial district in which Toolkot.com may be found.
- Your physical or electronic signature.
